Analyze, manage, and learn from patient safety incidents in any healthcare setting with the Canadian Incident Analysis Framework. Incident analysis is a structured process for identifying what happened, how and why it happened, what can be done to reduce the risk of recurrence and make care safer, and what was learned. It is an integral activity in the incident management continuum, which represents the activities and processes that surround a patient safety incident. The framework was developed collaboratively by CPSI, the Institute for Safe Medication Practices Canada, Saskatchewan Health, Patients for Patient Safety Canada (a patient-led program of CPSI), Paula Beard, Carolyn Hoffman, and Micheline Ste-Marie and is based on the 2006 Canadian Root Cause Analysis Framework. | Analysez, gérez et apprenez des incidents liés à la sécurité des patients dans tous les milieux de soins de santé, grâce au Cadre canadien d’analyse des incidents. L’analyse des incidents est un processus structuré pour identifier ce qui s'est passé, comment et pourquoi c'est arrivé, ce qui peut être fait pour réduire le risque de récurrence et rendre les soins plus sécuritaires, et ce qui a été appris. Il s'agit d'une activité qui fait partie intégrante du continuum de gestion des incidents, représentant les activités et les processus associés à un incident lié à la sécurité des patients. Le Cadre a été élaboré conjointement par l’ICSP, l’Institut pour l’utilisation sécuritaire des médicaments du Canada, Saskatchewan Health, Patients pour la sécurité des patients du Canada (un programme de l'ICSP dirigé par les patients), Paula Beard, Carolyn Hoffman, et Micheline Ste-Marie; et est basé sur le Cadre d’analyse des causes souches de 2006.
